About the Role 

The Clinical Team Leader position is a newly created position aimed at providing guidance, support and supervision in evidence based clinical practice with a small team of Care Advisors in the Home Care Package Program.  This includes applying program and professional knowledge in the management of escalated complex cases and secondary consults. You will also undertake Care Advisor responsibilities.

Some travel around metropolitan Melbourne may be required.

About You

To be successful in this role you will be consumer-centric and passionate about achieving high quality outcomes.

You will also have:

• Current AHPRA registration as Division 1 Nurse
• Knowledge and experience in contemporary evidence based clinical practice in a community setting
• Demonstrated experience and ability to lead supervise and motivate a team, ideally in a community setting.
• Strong computer skills: MS Word, Excel, PowerPoint, database, internet and Outlook and use of client management systems
• Evidence of Covid-19 vaccination, or medical exemption
• Willingness to undergo a National Police Check and have proof of the right to work in Australia
