Clinical Aged Care Assessor

  • Location – primarily remote and phone-based work  
  • Casual – ideally 3 days minimum availability between Monday and Friday (flexible with days and hours)
  • Pay rate – $54.47 per hour + casual loading (25%) + superannuation 
  • Pay less tax with Not For Profit Salary Packaging  
  • Travel will be reimbursed at 0.88 cents per km in line with our travel policy (if required for face to face assessments)

Our Aged Care Assessment Services (‘ACAS’) team is expanding, creating exciting opportunities! We are looking for compassionate and detail-oriented Clinical Aged Care Assessors. Current AHPRA registration and a Statement of Attainment 3 in Advanced Assessment Practice are required.

What will your role look like? 

In this role, you will be responsible for comprehensively assessing the health, functioning and support needs of our elderly clients using the IAT assessment tool. You’ll work closely with clients, their families and service providers to develop individual support plans, with recommendations for appropriate Home Care Package levels, Residential Respite and Permanent Residential Placement.

Day to day this includes: 

  • Collaborating with clients, families and multidisciplinary teams as required 
  • Working collaboratively with colleagues to promote a supportive and positive team culture 
  • Conducting accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and determining eligibility for home support services   
  • Reviewing support plans to meet client’s changing support needs 
  • Working effectively both autonomously and as a member of a broader team in delivering to key performance indicators within identified timeframes 
  • Maintaining currency of knowledge relating to aged care services both funded and non-funded 
  • Assisting with continuous improvement frameworks to regularly evaluate and enhance assessment processes and outcomes  

To be successful in this role you’ll need: 

  • Tertiary Qualification – in a health-related discipline directly related to Health, Aged Care or related specialist area (e.g. Registered Nurse, Medical Officer, Occupational Therapist, Physiotherapist or Social Worker) 
  • Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA)  
  • Statement of Attainment 3, Advanced Assessment Practice (essential)
  • An Australian Driver’s license and access to a registered, insured vehicle (preferred but not essential) 
  • Willingness to apply for a Police Check
