Social - Community Connector Lead
About the Role: Provide leadership for the Social Program, in addition to individual support to develop social plans and match customers to on- and off-site social experiences, in this full-time role based at Residential Care Home, Healthia, located in Elizabeth.
What you’ll do:
- Lead and mentor the social team to deliver social experiences
- Key care home contact for the social team and social experiences
- Create social plans and reviews that identify social goals
- Provide support and coaching to connect people to on and off site experiences
- Lead and run groups
- Assist in organising and running of events
- Orientate and supervise volunteers
About You!
You are talented and passionate about connecting people socially. You want to make the most of your expertise, do what you do best and thrive on seeing people have fun, engage in interests and connect with community.
- Experience in leading and facilitating groups and/or events and supporting participation of group members
- Capacity to work flexibly in partnership with customers and others using a strong customer service approach
- Computer skills to complete documentation to meet accreditation standards.
- Experience in the coordination of small to medium sized events
- Experience working with people at risk of social isolation and loneliness including those from a CALD background and people living with memory loss or Dementia
- Willingness to travel between residential care homes to backfill as part of the social team
You will need:
- Leadership / coaching experience
- Hold or working towards a Certificate IV Community Services / Aged Care or equivalent relevant qualification
- A satisfactory National Criminal History Record Check that complies with ACH Group Policy no older than 12 months at commencement of employment.
