The role: An exciting opportunity is available in the South/West and North/East metropolitan regions of Adelaide, for Registered Nurses with a passion for community nursing. Become an important member of the Community Nursing Team delivering services. Part time, minimum 3 days per week up to full time considered.
Be rewarded through working autonomously in this busy and exciting role. You will provide support to older people so they can remain independent and continue living in their own home.
What you'll do:
- Conduct assessments, reviews, coordinate care plans and nursing interventions for customers
- Support the utmost wellbeing of customers living in their own home
- Participate in on-call after hours service
- Liaise with internal and external stakeholders including family members, Support Workers and Allied Health Professionals to support customers
About you!
You are a talented and passionate Nurse. You want to make the most of your expertise, do what you do best and thrive on the challenge of providing our customers with quality care.
- Demonstrate significant experience as a registered nurse, preferably in community setting
- Excellent attention to detail
- Excellent interpersonal, communication and problem-solving skills
- Ability to work autonomously
- Intermediate computer skills
- Availability to work on call
You will need:
- A nursing qualification and current Registered Nurse registration with AHPRA
- To care about supporting older people to live a Good Life
- A current driver's licence and reliable vehicle
- A satisfactory police clearance for working with vulnerable people that is less than 12 months old before commencement
